You will closely collaborate with the patients, their families, and other members of the NLWC patient care team to provide and maintain continuity of patient care to achieve the patient's treatment goals that promote their overall wellness.
Main Job Functions Assess physical, environmental, and emotional factors to determine appropriate mental health and/or behavioral needs for patients.
Works in collaboration with the patient and caregivers along with NLWC treatment team to meet the emotional and/or behavioral needs of the patient.
Utilize a combination of NLWC resources and nationally recognized standards of practice to provide high levels of patient care services.
Actively participates in NLWC treatment team meetings in order to provide optimized quality care.
Essential Duties Conduct assessment for each patient receiving case management services and continue conducting assessments as needed based on the patients age or needs.
CANS/ANSA assessments must be done every 90 days for children/teens and 180 days for adults.
Develop an individual treatment care plan for each client following determination of needs and strengths.
Promote client's wellbeing by assisting in applying, advocating, or monitoring of any necessary community resources, services, or programs.
Provide skills training and psychosocial rehabilitative services to support client's development of independent living skills, social/communication skills, coping techniques, resilience, and overall life-domain wellbeing.
Render necessary communication, support, coordination, and monitoring of assigned caseload.
Conduct community-based services that emphasize client's desires for course of treatment.
Provide treatment and medication adherence education and support.
Ensure service delivery aligns with NLWC policy, regulation, and case management standards.
Ensure service data, progress notes, treatment plans, and other necessary documentation are entered within time requirements.
Maintain integrity and accountability during service-rendering and documentation, understanding organization's zero-tolerance policy.
Other duties as assigned by New Leaf Program Manager.
Job Eligibility Requirements Master's or a bachelor's degree in social work, psychology, public health, or related field and two (2) years of qualifying experience.
Qualifying experience means verifiable fulltime employment in a human services or related field.
CANS and ANSA certified and must maintain yearly certification.
Complete the state TMHP and MCO Case Management required trainings as needed.
Valid Driver's License and reliable transportation and recommended personal malpractice/liability insurance to complete home visits.
